    Facility Request Form Instructions:  Download the Facility Request form using the above link.  Complete the top portions of the first page only.  Submit facility request at least one week prior to the event to the building/facility principal (secondary cafeteria requests go to the junior high principal, fine arts building requests to the high school principal, and athletic facility requests start with the athletic director).  Please make arrangements for the unlocking and lock up of the facility as well as the need for custodians at the time of submission.
