Volunteers

Volunteer Criminal Background Check Form 
 All persons who wish to or have been asked to volunteer on a campus or as a student chaperone will be required to submit an application for a criminal background check.   Volunteers needing a background check would include, but may not be limited to, chaperones, field trip sponsors, and parent classroom volunteers. 

Forms should be turned in well in advance of the date the volunteer begins to allow time for the background check to be run. Background checks will not be run after hours due to the stringent protocols established by the Texas Department of Public Safety. 

Volunteers will need to fill out the form and have the appropriate administrator sign it. The administrator will email the completed forms to the district office. No one should go to the district offices to fill out or drop off a form.
